Are you an experienced Registered Nurse with a passion for clinical leadership and resident advocacy? We are currently seeking two dedicated Clinical Care Coordinators to join our teams at two premier residential aged care facilities in regional Victoria.
These roles are designed for high-achieving clinical leaders who are ready to drive excellence, mentor staff, and ensure the highest standards of care for our residents.
The Locations
We have two vacancies currently available. To be successful, candidates must be open to relocation within Victoria:
Position 1: Regional North-West Victoria.
Position 2: Coastal/Regional Vic
Key Responsibilities
Clinical Leadership: Provide expert guidance and mentorship to the nursing and care teams.
Care Coordination: Oversee complex care planning, resident assessments, and specialized clinical needs.
Quality & Compliance: Ensure all care delivery meets the Aged Care Quality Standards and maintain accreditation readiness.
Acuity Management: Monitor resident deterioration and lead clinical reviews and GP/NP rounds.
Operational Support: Support the Clinical Care Manager in auditing, KPI monitoring, and staff rostering/performance.

Mandatory Selection Criteria
To be considered for these roles, you must meet the following:
AHPRA Registration: Current, unrestricted registration as a Registered Nurse (Division 1).
On-Shore Experience: Extensive, proven experience working within Australian Residential Aged Care facilities.
Leadership Experience: Demonstrated ability to lead a clinical team, coordinate staff, and manage high-pressure environments.
Clinical Expertise: In-depth knowledge of AN-ACC, wound management, palliative care, and infection control.
Flexibility: Genuine willingness to relocate to regional Victoria.
