About the Role
We are looking for a dedicated Support at Home Care Partner to provide exceptional care and services that enable older individuals to remain safely at home. You will play a vital role in developing, implementing, and monitoring personalized support plans.
Key Responsibilities
- Work within a legislative framework to ensure compliance with the Support at Home Program.
- Guide Health Care Workers to deliver safe and timely care services.
- Actively identify and manage safety risks for participants.
- Collaborate effectively within a team and ensure high-quality service delivery.
- Communicate openly and participate in multi-disciplinary team meetings.
Minimum Qualifications
- Care Management Qualification (or currently working towards)
- Cert 4 Individual Support- with relevent expereince
- Current registration with the Nursing and Midwifery Board of Australia or similar
- Valid Victorian Driver's License
- Current Police Check
- Current AHPRA registration (if required)
- Influenza Vaccine (Current Year)
Key Selection Criteria
Essential:
- Strong time management and prioritization skills.
- Experience in individualized support within a person-centered framework.
- Ability to identify and manage participant safety risks effectively.
Desirable:
- Familiarity with software such as e-Tools, TRAKCare, RiskMan, and Microsoft Excel.
