The Opportunity
Are you a dedicated community professional passionate about helping others maintain their independence? We are seeking a Case/Care Manager to deliver person-centered, enablement-focused care. In this role, you will be the primary point of contact for our clients, ensuring they receive the support they need to thrive at home while adhering to the National Standards of Practice for Case Management and the National Code of Ethics.
Key Responsibilities
Case Management: Deliver evidence-based practice across various programs (HCP, CHSP, DVA, and Community Housing).
Care Planning: Conduct holistic assessments to develop collaborative, strengths-based Care Plans that respect client choice and decision-making.
Financial Oversight: Manage and monitor individualized client budgets, ensuring effective utilization of funds and resources.
Clinical Collaboration: Partner with Nursing, Allied Health, and Medical Practitioners to facilitate referrals and address complex clinical needs.
Reporting & Stewardship: Maintain accurate, up-to-date documentation and contribute to continuous improvement initiatives.
Community Engagement: Develop positive working relationships with local professionals and agencies to improve client outcomes.
Selection Criteria
To be successful in this role, you will need:
Qualifications: A relevant tertiary qualification in the community sector.
Sector Experience: Proven experience in case management within the aged, community, or disability sectors, particularly with complex needs.
Clinical Understanding: A commitment to reablement-focused care and culturally appropriate service delivery (CALD, ATSI, and Special Needs).
Financial Acumen: Understanding of individualized client funding and experience in budget monitoring.
Communication: Highly developed interpersonal and negotiation skills to engage with diverse stakeholders.
Technical Skills: Proficiency in Microsoft Office Suite and the ability to adapt to new databases.
Compliance: A valid Driver’s Licence and insured motor vehicle are essential.
