We are seeking an experienced Home Care Manager to lead our dedicated team in providing exceptional care to clients in Sydney and we also have a postion available in Brisbane. This managerial role offers the perfect opportunity for someone with a strong background in case management and home care to make a meaningful impact in the lives of our clients.
Key Responsibilities:
- Manage a caseload of up to 75 clients, ensuring the highest standards of care.
- Conduct weekly client visits to assess needs and monitor care plans.
- Oversee the administration of home care packages, ensuring compliance and quality service delivery.
- Collaborate with care partners and support staff to develop tailored care plans.
- Implement best practices and policies to enhance client satisfaction and operational efficiency.
- Provide support and mentorship to team members in the field.
Qualifications:
- Proven experience in case management or as a care partner within the home care industry.
- Strong knowledge of home care packages and relevant regulations.
- Previous aged care experience preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
