As a Care Representative, you will be the specialist who breaks down barriers for older people with complex needs. You will succeed by navigating the My Aged Care system with precision, ensuring that no one in our community is left to navigate their most vulnerable chapters alone.
Your Contributions: Based in Canberra, you will provide quality, person centred coordination that translates the organisation’s vision into reality.
- Navigate & Connect: Actively help clients navigate aged care systems and link them with essential community based services.
- Holistic Coordination: Organise and monitor individualised support pathways for clients with multiple or complex needs.
- Communication & Advocacy: Collaborate with families, carers, and third parties via phone and email to ensure service alignment.
- Operational Excellence: Maintain accurate electronic client records and contribute to the impact of the mission.
What You Bring to the Mission:
- Industry Insight: Sound knowledge of the aged care system and the My Aged Care process.
- Experience: Demonstrated experience working with older people facing significant barriers to accessing services.
- Qualifications: A Cert III or IV in Community Services, Aged Care, or a related field is highly regarded.
- A Caring, Empathetic Spirit: A person centred, culturally sensitive approach with the ability to build deep rapport and trust.
- Compliance Ready: A commitment to compliance with relevant legislation and mandatory training.
