Key Responsibilities
In this dynamic role, duties include:
Service Delivery & Coordination: Assist in implementing effective community transport services, including producing rosters and transport schedules.
Client Care & Review: Conduct regular reviews and update service plans in consultation with clients and their representatives to ensure ongoing individual needs are met.
Feedback & Collaboration: Provide vital feedback to the Team Leader regarding client issues, requests, concerns, and compliments.
Administration & Compliance: Maintain strict client confidentiality and ensure meticulous documentation within the management system to meet statutory and reporting requirements.
Records Management: Manage and maintain organisational records in accordance with strict policy procedures and practices.
About you:
- Experience in Home Support services and/or Aged Care is essential for this role.
- Willingness to Drive vehicle/transport Clients as required. You must hold a Current C Class licence.
- Working with Children Check – or Vulnerable Person-Related Employment Screening check required.
