About the Opportunity
We are launching an innovative, Nurse Practitioner-led dementia care program across our Residential Aged Care portfolio in the Central & Eastern Sydney (CESPHN) region.
As the NP Dementia Care Lead, you will serve as the advanced practice clinical authority, embedded within multidisciplinary teams to elevate the standard of complex dementia management. This is a high-impact role designed to reduce avoidable hospital transfers by uplifting the capability of onsite staff and streamlining coordination between GPs and hospital outreach services.
Key Responsibilities
Clinical Leadership: Deliver comprehensive NP-level assessments and management for residents living with dementia, including pharmacological and non-pharmacological interventions.
Collaborative Care: Coordinate case conferences and integrated care plans in partnership with GPs, Allied Health, and Local Health Districts (LHD).
Capacity Building: Mentor and coach Registered Nurses and care staff through targeted education to strengthen dementia health literacy.
Holistic Management: Oversee acute care episodes, chronic disease management, wound care, and psychotropic/deprescribing strategies.
Program Governance: Contribute to reporting and outcome tracking under the CESPHN funding agreement.
About You
To be successful in this role, you will be a clinical trailblazer with a passion for aged care reform. You must possess:
AHPRA Endorsement: Current registration as a Nurse Practitioner.
Experience: A minimum of 5 years of nursing experience, including at least 12 months in Aged Care with demonstrated expertise in complex behaviours.
Leadership Skills: A proven ability to mentor, inspire, and lead multi-disciplinary teams.
Cultural Competency: A commitment to providing culturally safe care for Aboriginal and Torres Strait Islander peoples and multicultural communities.
Logistics: A valid driver’s license and the ability to travel across multiple sites in the Sydney Central region.
