About the Opportunity
Our client is embarking on an ambitious new chapter in aged care, launching a Nurse Practitioner-led dementia care program across Residential Aged Care Homes (RACHs) within the Central & Eastern Sydney PHN (CESPHN) region.
As the Nurse Practitioner (Dementia Care), you won’t just be providing care, you will be the advanced clinical lead and architect of better outcomes. You will be embedded within multidisciplinary teams across 9 facilities, serving as the bridge between primary care, hospital outreach, and residential support. Your mission: to reduce avoidable hospital transfers, lead complex symptom management, and uplift the clinical capability of the entire care team.
Your Impact
Clinical Leadership: Deliver comprehensive NP-level assessments and management plans for residents living with dementia, utilizing both pharmacological and non-pharmacological strategies.
Capacity Building: Mentor and coach Registered Nurses and care staff, strengthening dementia health literacy and "uplifting" the quality of daily care.
Collaborative Care: Lead case conferences with GPs and Local Health Districts (LHD) to ensure seamless continuity of care and effective deprescribing.
About You
You are a bold, compassionate clinician who thrives on autonomy and clinical complexity. You meet the following:
Registration: Current AHPRA registration and endorsement as a Nurse Practitioner.
Experience: Minimum 5 years of nursing experience, including at least 12 months in Aged Care.
Expertise: Demonstrated mastery in dementia care and managing complex behaviors (BPSD).
Leadership: A natural mentor with excellent interpersonal skills and the ability to inspire professional growth in others.
Compliance: Current vaccinations, National Police Check, and a valid driver’s license for travel between sites.
Values: A deep commitment to culturally safe care for Aboriginal and Torres Strait Islander peoples and multicultural communities.
