About the Role
As a Travel Care Coordinator, you will be the clinical heartbeat of the facility. Working closely with the Facility Manager, you will lead the clinical team to ensure the highest standards of resident safety and well-being are met. These roles are perfect for clinical leaders who enjoy stepping into new environments, stabilizing teams, and making a rapid impact.
Key Responsibilities:
Clinical Leadership: Provide expert guidance and direction to the nursing and care staff.
Oversight: Manage clinical systems and processes to ensure compliance with Aged Care Quality Standards.
Case Management: Conduct assessments and work with families and allied health to develop tailored care plans.
Compliance & Audits: Ensure Commonwealth reporting requirements are met and conduct regular clinical audits.
Resident Safety: Monitor medication administration protocols and clinical data trends.
Key Requirements
Registration: Current AHPRA registration as a Registered Nurse.
Experience: At least 5 years in Aged Care, with proven leadership in a Clinical Coordinator or Care Manager capacity for at least 6 months.
Compliance Knowledge: Deep understanding of AN-ACC, Aged Care Quality Standards, and accreditation processes.
Travel-Ready Mindset: The ability to adapt quickly to new facilities and build rapport with staff and residents.
Documentation: Current National Police Certificate and up-to-date COVID-19 vaccination.
Licence: A current driver’s licence.
