The Opportunity
We are currently seeking a passionate and experienced Care Manager to join our operations team. In this role, you will be responsible for delivering case management that is truly person-centered and reablement-focused. Following established program guidelines and the National Standards of Practice, you will build positive relationships across the local service centre to facilitate the best possible outcomes for our clients.
Key Responsibilities
Holistic Care Planning: Assess client needs to develop collaborative, strengths-based Care Plans that facilitate client ownership of their own decision-making.
Funding & Budget Management: Manage individual client budgets effectively, demonstrating a strong knowledge of funding sources including CHSP, Home Care Packages, DVA, Respite Care, and Community Housing.
MDT Collaboration: Facilitate referrals and work closely with internal and external stakeholders, including Nursing, Allied Health services, and Medical Practitioners.
Monitoring & Review: Review client Care Plans and goals at agreed intervals, promptly monitoring and responding to any changes in client requirements.
Inclusive Service Delivery: Provide culturally appropriate care and services to CALD, ATSI, and other Special Needs clients.
Administrative Compliance: Comply with all reporting and data requirements to ensure accurate records while contributing to continuous improvement initiatives.
What You’ll Bring
Qualifications: Relevant tertiary qualification in the community, health, or aged care sector.
Sector Experience: Demonstrated experience in case management within the aged, community, or disability sectors—ideally working with clients with complex needs.
Financial Literacy: A practical understanding of the financial principles of individualized client funding and experience tracking individual budgets.
Communication & Collaboration: Highly developed communication, negotiation, and interpersonal skills to collaborate effectively with internal teams, families, and external agencies.
Tech Literacy: Highly developed proficiency using computers and the Microsoft Office Suite.
Compliance: A valid Driver’s Licence and a registered, insured motor vehicle.
