Key Responsibilities
Oversee and manage complex Home Care Packages for clients living in the community.
Conduct clinical assessments, care planning, and ongoing reviews.
Deliver advanced clinical care, including high-care needs such as complex wound management and catheter care.
Liaise with multidisciplinary teams, clients, and families to ensure high-quality, person-centered outcomes.
Requirements
Bachelor of Nursing degree.
Current, unencumbered AHPRA Registration as a Registered Nurse.
Demonstrated clinical experience in complex nursing care (wound care, catheter management, etc.).
Prior experience in home care, aged care, or community nursing preferred.
Valid Australian Driver’s Licence and Rights to Work in Australia.
