In this role, you will deliver comprehensive, client-centered nursing care to individuals in their homes and community clinic settings. You will play a vital role in promoting independence, managing complex chronic conditions, facilitating wound care, and delivering palliative care across our regional community.
Key ResponsibilitiesConduct holistic clinical assessments and formulate care plans tailored to individual client needs.
Deliver high-quality nursing care in client homes and clinic settings, including wound management, medication administration, chronic disease management, and palliative care.
Collaborate closely with GPs, allied health professionals, acute health services, and community care teams.
Maintain accurate, timely, and compliant electronic medical documentation.
Educate and support clients, families, and carers to manage health conditions independently and safely at home.
AHPRA Registration: Current registration as a Registered Nurse (RN) or Enrolled Nurse (EN) with the Nursing and Midwifery Board of Australia.
Community Nursing Experience: Essential. Proven experience delivering autonomous nursing care within community, home care, or district nursing environments.
Driver’s Licence: Current Victorian Driver’s Licence (access to a pool vehicle or vehicle allowance provided).
Certifications & Checks:
Current National Police Check (or willingness to obtain).
Working with Children Check (WWCC) & NDIS Worker Screening Check (if applicable).
Current First Aid and CPR certifications.
Strong clinical reasoning, time-management, and problem-solving skills with the ability to work independently in the field.
