We are seeking Expression of Interests from qualified candidates in program management of crisis support referrals. This role incorporates on the phone crisis triage, referrals, reporting and providing support to access the NDIS. This is a unique opportunity for suitable and qualified candidates to have direct and immediate impact on individuals lives.\

Working within a small team who act as the initial, primary access point for the risk assessment of/and response to individuals who may be seeking immediate help.

All communication takes place via phone and email, as this role is the single or primary point of contact and access to clinical support for those seeking help for perhaps the first time. As such, this fast-paced role would ideally suit an experienced Mental Health professional who can think on their feet, remain calm and be patient with those in need at all times while leading a team and having management/TL experience.

The Role:

As the Program Manager - Crisis Referral, you will be responsible for the management of a team of Crisis Support Coordinators as well as be hands on with the assessment and response to clients who have urgent/emergency referrals. This process involves speaking with clients, conducting risk assessments, identifying barriers, and discussing broader supports and needs, as well as services or entitlements available from the NDIS.

Your duties would include (but not be limited to)

  • Provide direct crisis referral support and support coordination responses in a timely and efficiently.
  • Undertake all aspects of direct line manager of direct staff including

- Perform ongoing supervision for direct staff

- Provide leadership and guidance

- Manage training and professional development of team

  • Provide guidance and assistance to staff when dealing with complex situations and incidents
  • Respond to and manage referrals with efficiency and good client outcomes
  • Implement a risk management approach to service delivery and ensure staff adhere to legislation, policies and procedures.
  • Implement a continuous improvement framework to better policies, procedures, work instructions, other organisational systems, and the work environment.
  • Participate and facilitator on-call where required.
  • Complete quality and accurate reporting within required deadlines
  • Collaborate with relevant stakeholders to prepare, undertake and maintain external licensing/accreditation requirements.

About You:

  • Tertiary qualifications in Allied Health, Community Services or equivalent extensive experience.
  • Extensive experience within the complex case management and/or case management role
  • Proven experience managing staff and effectively running a team
  • Previous experience with NDIS Support Coordination is desired
  • Strong understanding and experience within NDIS space
  • Excellent written and verbal communication skills with high attention to detail
  • Queensland Provisional Green or above Driver Licence.
  • Ability to obtain positive notice in the legislatively required personal history check in line with service stream compliance requirements.
