The Role:

Working closely with the Assessment and Care Planning team, and ACFI Manager, the Clinical Nurse CAD, will be responsible for the comprehensive contemporary assessment and care planning for all residents, enabling each resident’s quality of life and independence to be restored and maintained at an optimum level.

Your responsibilities:

  • Ensure high quality standardised processes in assessment and care planning and ACFI management, with the support of a diverse and knowledgable team.
  • Undertake comprehensive assessment and care planning for all new residents and residents with significant change in health status.
    Collaborate and ontribute to ongoing evaluation of care to enable resident’s quality of life and independence.
  • Lead and influence the delivery of care through assessment and care planning practices
  • Researching and role modelling evidence based best practice and coordinating comprehensive assessment and care planning.
  • To maximise care subsidy income and ensure appraisals packs support validation requirements through application of the Aged Care Funding Instrument (ACFI)
  • Work with the ACFI Consultant to manage the validation process - and maintain current knowledge of changes
  • Training and mentoring of staff, ensuring they understand why CAD and ACFI are important and relevant
  • Meet all KPI's and benchmarks
  • Continuously look for improvement opportunities, including looking and research and policy development to ensure they remain to be seen as a leading, forward thinking and ethical organisation.

The requirements:

  • Current AHPRA registration as a RN
  • Thorough knowledge and experience with assessment and care planning
  • ACFI knowledge / experience
  • Previous experience as a Clinical Nurse or management role
  • Excellent communication and relationship building skills
  • Highly organised with excellent administration skills
  • Passion for making a difference in day to day lives of residents through
  • Aged care experience as a RN or excellent knowledge of the aged care sector
