job description

A fantastic opportunity has become available for a suitably qualified mental health professional (Social Worker/Occupational Therapist/Psychologist/Registered Nurse) working with a leading Government Department, based in Sydney CBD. The aim of this program is to provide specialised, goal-oriented case management to contemporary veterans and dependants identified as having complex and multiple needs. As a Clinical Support Case Manager you will manage your own caseload in order to ensure continuity of care of clients.

ABOUT YOUR ROLE:

  • OVERSEE a caseload of clients that have complex needs
  • PROVIDE clinical risk assessments for identified clients
  • MANAGE escalations to internal senior managers and external agencies where significant risk is identified
  • COMPLETE appropriate documentation regarding process, nature and outcomes of risk assessment
  • COLLABORATE with staff members on request on areas of subject matter expertise
  • DELIVER case management for clients of the Client Support Framework

ABOUT YOU:

  • Qualifications and/or relevant registration with AHPRA as a Psychologist, Registered Nurse (Mental Health),Occupational Therapist, or Social Worker (AASW).
  • Working with Children Check & Vulnerable peoples check
  • EXTENSIVE experience within the clinical case management space (2 + years)
  • Defence and/or Veterans Affairs knowledge (distinct advantage)
