This is an urgent, 4- 8week temporary opportunity.

Our client runs a highly-regarded Support at Home program with 90 clients on their books. What truly sets this team apart is the incredibly friendly and collaborative culture. You'll be joining a service where Care Partners regularly discuss cases, share challenges, and work together to brainstorm the best solutions for their clients. Your input will be valued, and you'll always have team support.
📝 The Role: What You'll Be Doing
We are seeking experienced Care Partners to join the team temporarily to help ensure all client agreements and care plans are up-to-date and compliant. Your key focus will be on ensuring safe, timely, and individualised care for all participants.
- Client Review & Assessment: Organise and conduct in-home care visits to review clients' capacity, risk profile, and current needs.
- Care Plan Management: Ensure the client has the correct home care package/agreement and level of care in place, developing, implementing, and monitoring individualised support plans.
- Compliance & Documentation: Accurate documentation and assessments following client visits, adhering to clinical governance frameworks and Aged Care Quality Standards.
- Team Collaboration: Work cohesively with Health Care Workers, Allied Health, and other providers to ensure coordinated, holistic care delivery.
Stakeholder Communication: Liaise with participants, their families, and external suppliers/providers.

Essential Qualifications
The ideal candidate will hold one of the following qualifications:
- Diploma or Bachelor of Nursing (Current AHPRA registration required).
- Diploma in Community Services.
- Certificate IV or V in Ageing.
- Minimum: Certificate III in Aged Care with extensive, demonstrable experience in a similar Case Management role.
Essential Experience & Attributes
- Prior Care Partner, Case Management, or Care Coordinator experience in a similar role within Aged Care or equivalent, managing care plans and assessing risk.
- Passionate about aged care and committed to a person-centred framework.
- Excellent time management, prioritisation, and verbal/written communication skills.
- Proficient IT skills (Microsoft Word, Outlook) with the ability to quickly learn new database systems.
- A true team player—collaborative, empathetic, and professional.
Required Checks & Licences
- Current Police Check
- Valid Victorian Drivers Licence/fleet vehicle available for client travel
- Current Influenza Vaccine
- Current AHPRA registration (if applicable to qualification)
