The purpose of the Care Manager role is to manage the quality of the care delivered and develop a network of community contacts for the development of providing community services. The ultimate measure of the success of this role is the level of customer and carer satisfaction and the impact on the community through a network of relationships.

1. Create networking opportunities.
2. Attend networking events.
3. Convert new clients.
4. Conduct weekly client reviews.
5. Perform Home assessments on clients.
6. Perform Care assessment on clients.
7. Develop Care Plans for Clients.
8. Manage Client Issues.
9. Manage Carer Issues.
10. Assist with Recruitment Carers and Registered Nurses.
Qualifications and Requirements
- Enrolled Nurse, Occupational Therapist, or Registered Nurse qualification.
- Previous experience in home care, aged care, or community care management is highly desirable.
- Strong organizational and communication skills.
- Ability to work independently and manage multiple tasks efficiently.
- Willingness to travel as required.
- Valid driver’s license.
